Quick Answer
In the first 24 hours after penis filler, apply cool compresses, take acetaminophen if needed, sleep on your back, and avoid alcohol, NSAIDs, and any pressure on the area. Days 2 to 7 are about minimizing inflammation and avoiding heat exposure. Weeks 2 to 4 are when you cautiously return to sex and exercise. Most NYC patients are at full activity by day 14. Call 866-477-2023 immediately if you notice severe pain, dusky discoloration, or worsening swelling after day 4.
Penis filler is an in-office procedure with no general anesthesia, no surgical incisions, and no recovery in a hospital bed. The aftercare is simple, but the rules matter. Following the protocol closely in the first two weeks is what separates a smooth recovery from one with bruising, asymmetry, or filler displacement.
This guide is the same aftercare protocol Dr. Elliot Heller’s NYC patients receive on their procedure day, in writing, with reasoning attached so you understand why each rule exists. The protocol applies to standard hyaluronic acid filler treatments performed at the Manhattan, Staten Island, and New Jersey offices.
The first day after treatment is when the tissue is most sensitive to disruption. The filler is in place but has not yet integrated. Strict adherence in this window pays off for the entire result.
Do:
Avoid:
Most patients report mild soreness comparable to a deep bruise rather than sharp pain. If pain is severe or escalating, that is not normal and warrants a call to the office.
Days 2 and 3 are typically when swelling peaks. This is normal and expected. The penis will look noticeably larger than your eventual stable result during this window.
Continue avoiding:
Cleared activities by day 2:
By day 7 the visible swelling has typically resolved by 60 to 80 percent. Bruising, if present, has faded to yellow or is gone entirely. You should feel essentially normal, with the treated area returning to a more natural feel as the initial post-injection firmness softens.
The two-week mark is the standard cutoff for resuming normal activities, including penetrative sexual activity. By this point, the filler has begun integrating with surrounding tissue and the risk of mechanical displacement is much lower.
Cleared at week 2:
Patients often ask about the “first time” after filler. The answer: gentle, well-lubricated, and your partner should be aware that you had treatment so they can adjust accordingly. There is no benefit to rushing back at day 8 instead of waiting until day 14, and there is meaningful downside if anything goes wrong during integration.
By the 30-day mark, the following should be true:
If any of the above is not true at the 30-day mark, schedule a follow-up visit. Most concerns at this stage have a simple explanation and a simple fix.
Dr. Heller’s perspective: “The patients who follow the aftercare carefully are the ones who never call us with complaints. The patients who try to compress the timeline (back at the gym day 3, sex day 5, hot tub day 7) are the ones who introduce avoidable problems. The protocol is short. Just follow it.”
Penis filler is delivered through tiny puncture sites that close within minutes of injection. There are no stitches, no surgical wounds, and no dressings. Hygiene is straightforward:
Trimming or grooming pubic hair: hold off for 7 days to reduce the risk of nicks near the injection sites. After day 7, normal grooming with a clean trimmer is fine.
Helpful in the first 7 days:
Avoid in the first 7 days:
Pause any supplement that has a known blood-thinning effect for at least 5 days post-treatment. Resume normally after that.
Most patients have an entirely uneventful recovery. The following symptoms are not normal and warrant a call to the office immediately:
The Manhattan office line is staffed during business hours. After-hours urgent calls route through a protocol that gets you in contact with Dr. Heller or covering staff. Call 866-477-2023 with any concerns. The earlier a problem is identified, the simpler the fix.
Penis filler aftercare follow-ups are available at all three of Dr. Heller’s offices. The standard protocol is a brief check-in around the 4-week mark to review the stable result and answer questions. Patients regularly travel from Upper East Side, Midtown East, Murray Hill, Lenox Hill, Tribeca, Park Slope, and Williamsburg, as well as from across Long Island and northern New Jersey.

Avoid ibuprofen and other NSAIDs for the first 7 days. NSAIDs have mild blood-thinning effects that can worsen bruising. Acetaminophen (Tylenol) is the recommended pain reliever during the recovery window.
Walking and light cardio are fine from day 1. Weight training and moderate cardio resume around day 7. Heavy lifting and intense cardio resume at day 10 to 14. Cycling, particularly long road rides, should wait until day 14 to 21 to avoid prolonged pressure on the treated tissue.
Solo activity is generally cleared at day 7, gently. Avoid aggressive friction or unusual positions for the first session. Penetrative sex with a partner waits until day 14.
Alcohol is a vasodilator that increases blood flow and worsens both swelling and bruising in the post-injection window. Avoid for the first 5 days, then resume in moderation.
Small areas of firmness are normal in the first 1 to 2 weeks as the filler integrates. They typically soften and become indistinguishable from the surrounding tissue. If a discrete lump persists past 4 weeks, schedule a follow-up. Persistent nodules can be addressed with massage, additional treatment, or in rare cases dissolution.
Wait 14 days for swimming pools, hot tubs, saunas, and any extended hot water exposure. Heat dilates blood vessels and can worsen any residual swelling. Brief warm showers are fine starting day 1.
