Penis Enhancement Surgery by New York Plastic Surgeons

Penis enlargement surgery, known as “phalloplasty,” is performed to enhance the length and width of the penis. This is a popular procedure that is frequently performed in our New York Surgical Center.

The majority of patients will choose to have penis lengthening or widening performed alone or together in a single session. Glanular and scrotal enhancement can also be added to penis enlargement or performed alone to meet the individual’s unique needs and desires.

Penile Lengthening Surgery

Up to one-half of the penis is located in the body in a curvature, anchored to the pubic bone by the fundiform and suspensory ligament. By releasing the ligaments with an incision hidden in the pubic hair region, the penis will protrude on a straighter path with a one to two inch increase in functional length. Maximum length gain is achieved when patients comply with using a tension weight device, which we highly recommend. Penile lengthening procedures can be performed alone or along with widening and scrotal enhancement.

Penile Widening Surgery

Widening of the penis to increase the girth (circumference) is performed in one of several ways.

Alloderm® Matrix is a thick sheet of tissue which is placed under the skin of the penis to achieve a width gain of up to 25-35 percent. Dr. Heller has pioneered the double layering technique for maximum gains in penile enlargement surgery. Procedure incisions heal quickly and require a downtime of only 1-2 days.

Alternatively, the penis can be widened using the fat transfer method where fat is taken from the abdomen or flanks and injected beneath the skin of the penis to increase the thickness. No incisions and no stitches are required for this male enhancement surgery, which can be performed while you are awake. Results of this male enhancement are immediate and normal activities can be resumed the following day. Harvested fat can be stored and used for subsequent procedures to achieve a quite large increase in penile length and width.

A fat transfer can be performed as a procedure by itself for men looking for a quick, lower cost way to achieve penile enlargement.

Secondary Penile Widening

Sometimes patients who have received penile widening or male enlargement, via liposuction/lipotransfer (fat injection) or autograft dermal fat graft or Allograft Dermal Matrix Graft (Alloderm®) desire additional girth enhancement.

Subsequent fat transfers can always be used to enhance width when the patient has chosen to leave some fat in cold storage from their previous penis enhancement.

Alloderm® techniques can be used to insert Allograft Dermal Matrix Grafts above and below a previously inserted graft with excellent results in patients who have had previous widening procedures.

Dermal Fat Graft

A new technique for widening the penis girth is performed by removing a small portion of skin and fat from beneath the buttocks. This is placed under the penile skin and allows the penis to become much thicker. The donor scar is imperceptible in the crease below the buttocks. This is a good technique especially for those patients that dont have enough fatty tissue on their stomach or love handles for liposuction. The downtime is minimal and there is hardly any discomfort.

Glanular Enhancement

Glanular enhancement involves increasing the glans (head) of the penis and is a relatively new surgical technique for male enlargement. Very few surgeons offer this procedure, however, Allograft Dermal Matrix Graft (Alloderm®) technique yields satisfactory results of up to a 30 percent increase in the flare of the glans with an average increase that falls within 15–20 percent.

When performed in addition to penile widening, no additional incision is required and a penile stretching device isn’t necessary to achieve greater results.

Scrotal Lift

Male scrotal skin becomes more lax with age. The skin of the scrotum is a complex structure that contains smooth muscle, which allows contraction and relaxation of the area, depending on the surrounding temperature. Often the skin becomes excessive and the scrotum hangs too low, or becomes saggy and redundant.

Many men feel uncomfortable with the loose skin on their scrotum and the appearance of low hanging testes.

A scrotal lift can remove the loose skin and tighten the scrotal sac, resulting in a more youthful look. The incisions are hidden and recovery is fairly quick- usually 1-2 days. This procedure is an excellent addition to male enlargement surgery in mature men.

Scrotal Web Release

Scrotal Web Release is also performed quite frequently to separate the attachment of the under-surface of the penis to the scrotum, which can result in a shortened appearance. This procedure makes the penis appear longer and not tethered to the scrotal skin.

These procedures are performed as a same-day out-patient surgery for a quick recovery and can be added to your male enlargement surgery. All sutures used are dissolvable, with no stitches requiring removal.

Pubic Lift and Liposuction

Whenever there is a large pad of fat in front of the pubic bone, it has a tendency to engulf or hide some of the penile shaft. In order to maximize the results of penile lengthening, this fat pad should be removed. This can be done surgically by making an incision and removing the extra fat and skin, or by performing liposuction. The need to perform this procedure will be determined during a thorough examination for your penis enlargement surgery.


Circumcision is a procedure that has been shown to reduce infection in males, in addition to decreasing the risk of cervical cancer in female partners. Men should be circumcised if they have recurrent infections of the foreskin or if the foreskin cannot be fully retracted. Some men choose to have circumcision for purely cosmetic purposes.

Adult circumcision is performed by making an incision in the outer and inner surfaces of the foreskin and removing the excess skin. Often, a circumcision will be recommended during the male enhancement surgery to ensure that the Alloderm® is properly placed. These procedures can all be done at the same time as other penile enhancements to minimize cost and discomfort.

Non-Surgical Enlargement
